The foundation was incorporated in 1984. It serves as a source of contributed funds for West River Health Services and Western Horizons Living Centers. The foundation is governed by a volunteer board of directors and is a registered 501 © (3) U.S. charitable organization and licensed by the state of North Dakota.

To support West River Health Services and Western Horizons Living Centers as they provide comprehensive health and wellness services to the region by providing fund raising and development services.
